Exploring the path to elimination in the UK and the worldwide evidence behind moving to a one-dose vaccination schedule.
Almost all cervical cancers are caused by HPV (human papillomavirus) infection. Following the development of effective prophylactic HPV vaccines, the World Health Organization (WHO) launched a global initiative to “eliminate” cervical cancer through a three-pronged approach: vaccination, screening, and treatment. Elimination is defined as cervical cancer incidence of fewer than 4 per 100,000 women per year, a target the NHS has pledged to reach by 2040. Dr Clare Gilham and Dr Kathy Baisley from the Faculty of Epidemiology and Population Health will present their work contributing to this global aim and discuss the facilitators and barriers to reaching the target.
Dr Clare Gilham will give a short introduction to HPV and cervical cancer, outlining the WHO targets for elimination and will show the impact of HPV vaccination and cervical screening on cervical cancer incidence and mortality in the UK. She will also present some results from her latest project which is offering a catch-up HPV screening test to women aged 60-79 who were too old to be offered primary HPV screening by the NHS programme when it was introduced in 2019.
The global move to single-dose HPV vaccination is one of the most significant recent shifts in immunisation policy. Dr Kathy Baisley will review the evidence behind that change, from randomised trials and observational studies through to the current landscape of national programmes, and will highlight the key key questions that remain. She will then present three LSHTM-led trials advancing this agenda: DoRIS, the first randomised trial of a single dose in the target age group; Add Vacc, a cluster randomised trial of gender-neutral single-dose vaccination of adolescent males in Tanzania; and RIFT-SSA, a proof-of-concept study testing whether vaccination of HPV-positive women reduces viral infectivity.
