W4 follow up questionnaire 2010 (PDF 0.51 MB) A fourth self-administered postal questionnaire was sent in 2010 and 2011.
W4 activity diary (PDF 0.47 MB) Together with the W4 follow up questionnaire this activity diary was sent out as part of the data collection.
Follow up Questionnaire 2007 (PDF 0.13 MB) -A third self-administered postal questionnaire was sent in 2007. This was about health and lifestyle to enable us that see what was changed, and what has stayed the same since our last survey in 2003.
Follow up Questionnaire 2003 (PDF 1.25 MB) - A self administered questionnaire about lifestyle and medical history which was sent out in March 2003.
Town profiles - A summary of the profiles of the 23 towns derived from town data collected at he baseline examination 4286 women in 1999-2000.
Study protocol - The design of the British Women's Heart and Health Study, including details of all tests and techniques used.
Baseline Questionnaire (PDF 0.11 MB) - A self administered questionnaire about lifestyle and medical history used at the baseline visit.
Interview Questionnaire (PDF 0.03 MB) - Interview questionnaire. A nurse-administered questionnaire asking in more detail about any existing heart problems/symptoms, and recording current medication at baseline.
Data Sheet (PDF 0.01 MB) -This document is used to record the results of all of the tests carried out at our screening sat baseline.
Blood regimen-Finally there is a summary of the Protocol (DOC 0.11 MB) for obtaining and aliquotting blood samples for all of the laboratory tests.
All data collected is held by the research team based at London School of Hygiene and Tropical Medicine, for ongoing analysis.
